Promotes safe and effective personal care in the patients' place of residence by assisting with personal hygiene needs, ambulation and/or activities of daily living under the direction of the Home Health or Hospice Case Manager. Contribute to the comfort and dignity of patients and provide care that is consistent with patient needs and goals. Serves as an ambassador for all Home Options departments.


• Promotes a safe and therapeutic environment for the patient while respecting the patient's home and personal property.
• Demonstrates the ability to work independently as appropriate. Requests additional instruction or clarification as needed.
• Demonstrates dependability, responsibility, and flexibility with the scheduling process and confirmed working schedule.
• Assists with the care of the dying patient. Is aware of the family/caregiver or other individuals living with the patient and interpersonal issues.
• Informs Home Health Team Leader/Supervisor or designee of pertinent changes in client status.
• The employee demonstrates competency in essential (high volume, high risk) job functions.
• Attend IDT/HH Team meetings
• Provides appropriate patient care skillfully as outlined on the Aide Assignment sheet and according to department policies.
• Demonstrates independent and organized proficiency as a HHA Demonstrates proficiency in basic personal care skills: bathing, hair care, grooming, ambulation/exercise, ADLs, etc., and other specialized tasks.
• Demonstrates proficiency in all housekeeping functions: meal prep, food storage, and light housekeeping to maintain a clean environment, etc



• Current Montana Nurse Aide certification
• Current Home Health Aide certification.
• Current CPR/AED certification required.
• Reliable vehicle, current Montana Drivers License and a minimum of liability insurance.
• Home Health and Hospice experience preferred Job Duties:
• Completes home visit competency successfully.
