  • Overview

    This position follows a patient specific goal directed Plan of Care. Collaborates with clinical team and participates in the planning and evaluation of patient care. Services adult and at times, pediatric populations. Assists and supports the department team as needed.


    1. *Provides skilled nursing care to patients through the use of the nursing process as appropriate for LPN’s in the homecare setting.
    2. *Contributes to maintaining an updated, individualized plan of care that includes; goals, interventions, medications and visit frequency orders.
    3. *Performs and documents physical, psychosocial and spiritual assessment of assigned patients using all available sources of information. Evaluates and documents the patient’s perception of illness/condition.
    4. *Performs medication reconciliation during visits.
    5. *Responds to changing workload in a positive and professional manner including, but not limited to; remaining calm, making objective assessments, prioritizing and asking for assistance when appropriate.
    6. *Transcribes and implements physician orders and treatments, both verbal and written, accurately and efficiently.
    7. *Consistently problem- solves issues affecting patient care and reports instances to leadership.
    8. *Maintains a thorough knowledge of infection control procedures and strictly adheres to policies and procedures.
    9. *Demonstrates an understanding of and adherence to Home Health/Hospice CMS guidelines and Conditions of Participation.


    1. Licensure as required by guidelines.
    2. Minimum of two (2) years’ experience in similar position with a medical setting. Medical or surgical experience preferred.
    3. Must possess knowledge of professional nursing theory to give and evaluate patient care. Possess knowledge of basic skilled nursing procedures and medical terminology.
    4. Current BLS certification required or must successfully obtain within sixty (60) days of hire..
    5. Experience with equipment, medical supplies and wound care preferred..
    6. Ability to make independent judgments under stressful situations at times. Must work effectively and efficiently without direct supervision.
    7. Montana Drivers license, have a good driving record clear of moving violations in the past five (5) years and no major driving offenses (DUI, etc.).
    8. Must have capability to connect to internet for daily data exchange.
