  • Overview

    Provides safe and effective personal care in the patients’ residence by assisting with personal hygiene needs, ambulation and/or activities of daily living under the direction of the Home Health or Hospice Case Manager.  Contributes to the comfort and dignity of patients and provides care that is consistent with patient needs and goals. Serves as an ambassador for all Home Options departments.


    1. *Successfully completes home visit competency.
    2. *Promotes a safe and therapeutic environment for the patient while respecting the patient’s home and personal property.
    3. *Works independently under the supervision of a Registered Nurse.
    4. *Exhibits dependability, responsibility, and flexibility with the scheduling process and confirmed working schedule.
    5. *Assists with the care of the dying patient and is respectful to family/caregiver or others and interpersonal issues.
    6. *Informs Home Health Team Leader/Supervisor or designee of pertinent changes in patient status.
    7. *Exhibits competency in essential (high volume, high risk) job functions.
    8. *Provides appropriate patient care skillfully as outlined on the Aide Assignment sheet and according to department policies.
    9. *Proficient in in basic personal care skills to include, but not limited to;  bathing, hair care, grooming, ambulation/exercise, ADLs, and other specialized tasks.
    10. *Proficient in all housekeeping functions to include, but not limited to; meal prep, food storage, and light housekeeping to maintain a clean environment, etc.
    11. *Performs as a caregiver in all Home Options departments


    1. Current Montana Certified Nurse Aide (CAN) certification.
    2. Current Home Health Aide certification preferred.
    3. Current HS AED CPR Certification preferred or must successfully obtain within 30 days of employment.
    4. Must possess a valid driver’s license; a good driving record clear of moving violations in the past five (5) years and no major driving offenses (DUI, etc.). Must possess and maintain current insurance on vehicle.
    5. Home Health and Hospice experience preferred.
    6. Must possess professional communication skills, and work effectively with a diverse customer and coworker population.
